Permit/license information

Going to get your permit

During the first few classes, we will focus on learning the basic rules of the road and preparing for the written permit test. 


After the completion of the second class, students will be eligible to obtain their permits. Students will be instructed to bring the following information to the Driver's License Facility when taking the written permit/vision screening test. However, students will not be given the permit application until classroom fees are paid in full.


  • Permit Application Form 
  • Original Birth Certificate
  • Photo ID
  • Social Security Card
  • $20
  • Parent or Guardian w/valid driver license
  • Proof of Residency (2 forms)

The following information is given to each student, once we receive a copy of their permit. 


  • Student Driver window cling (while supplies last from the state)
  • Illinois Graduated Driver Licensing Program pamphlet
  • Reminder information regarding scheduling Behind-the-Wheel

It is required by All Four Wheels that we obtain a copy of your permit for our records.

Going to get your license

After completing Behind the Wheel and/or turning 16, students will be eligible to obtain their driver’s license. Students are instructed to bring the following documents to the Department of Motor Vehicles.


  • Permit
  • Original Birth Certificate
  • Photo ID
  • Social Security Card
  • Insurance Card
  • Vehicle Registration
  • Proof of Residency (2 forms)
  • 50-hour log in sheet


Students that have completed the classroom portion but not their Behind-the-Wheel and are 18 years or older, MUST take their high school transcripts or Letter of Completion, showing that you have taken and passed the classroom portion of Drivers Ed. Otherwise, they will send you home.
